Terms and Conditions for In-Home Diagnostic Services

1. Introduction
Welcome to BlackPoint Diagnostics. By booking our in-home diagnostic services, you agree to comply with and be bound by these Terms and Conditions ("Terms"). These Terms govern your use of our services, including all bookings, payments, and interactions related to health screenings, diagnostic tests, and any other services provided by Precision Diagnostics. Please read these Terms carefully before confirming your appointment.
2. Scope of Services
BlackPoint Diagnostics provides non-invasive health screenings and diagnostic services, including but not limited to blood pressure monitoring, cholesterol and glucose testing, echocardiograms, and other vascular and cardiac health assessments. Our services are intended for general wellness purposes and to provide you with essential health information. These services are not designed to diagnose or treat specific medical conditions but to detect potential health risks.
3. Eligibility and Client Responsibilities
By booking an appointment, you confirm that you are at least 18 years old, or you are booking with the consent of a legal guardian if under 18.
You agree to provide accurate and truthful information about your health status during the booking process and any intake forms. You understand that failure to disclose relevant medical conditions, allergies, or medications could impact the accuracy of your results and your health.
4. Appointment Booking, Confirmation, and Payment
Appointments must be booked in advance via our website or through authorized booking channels.
Full payment for services is required at the time of booking to confirm your appointment. We accept payments through credit/debit card or other authorized payment methods.
Once your appointment is confirmed, you will receive an email with a detailed confirmation, including the date, time, and location of your scheduled in-home visit.
5. Rescheduling and Cancellation Policy
If you need to reschedule or cancel your appointment, please provide at least 48 hours’ notice.
Cancellations made within 24 hours of the scheduled appointment will incur a 50% cancellation fee of the total service cost.
"No-show" appointments, or failure to attend the scheduled appointment without prior notice, will be subject to a charge equal to the full cost of the scheduled service.
In case of extenuating circumstances, please contact us as soon as possible to discuss possible solutions.
6. Client Preparation for In-Home Services
Please ensure that the location for the in-home visit is clean, quiet, and suitable for the screening. A private area with access to electricity is required for certain equipment.
You are responsible for being available at the scheduled time. If there is a change to the appointment, please notify us immediately.
It is important to review and complete any health intake forms before the visit. These forms must be submitted in advance to allow our team to tailor the service to your needs.
If you are unable to provide accurate or complete health information or are unprepared for the appointment, our staff may be unable to proceed with the screening.
7. Confidentiality and Data Privacy
BlackPoint Diagnostics is committed to maintaining the confidentiality of your health information in accordance with all applicable privacy laws and regulations.
We collect, store, and use your personal and health information only for the purpose of providing our services, conducting diagnostic tests, and supporting your overall wellness.
By booking and using our services, you consent to the collection and use of your personal data as outlined in our Privacy Policy. We will never share your information with third parties without your express consent, except as required by law or for healthcare purposes.
8. Results and Interpretation
All diagnostic results are reviewed and interpreted by board-certified medical professionals to ensure accuracy.
Results will be shared with you within the timeframe specified at the time of service. You will receive a detailed report of your results, along with guidance on the next steps if necessary.
Please note that our services are not a substitute for regular medical care. If any results suggest potential health concerns, we recommend that you follow up with your primary care provider or specialist for further evaluation and treatment.
9. Limitations of Liability
BlackPoint Diagnostics is committed to providing quality services, but we do not guarantee specific health outcomes or results. The information provided during our screenings is based on available diagnostic methods, but may not detect all health issues.
You agree to hold BlackPoint Diagnostics, its employees, contractors, and agents harmless from any claims, losses, or damages resulting from the use of our services, including but not limited to misinterpretation of results, health complications, or missed diagnoses.
BlackPoint Diagnostics is not liable for any health conditions, injuries, or complications that occur before, during, or after the provision of our services.
10. Consent for Service
By booking an appointment, you acknowledge that you have read, understood, and agree to these Terms. You confirm that all health information provided is accurate to the best of your knowledge and that you are voluntarily seeking our services. You also consent to the collection, use, and sharing of your personal data as necessary for the provision of the service.
11. Modifications to Terms and Conditions
BlackPoint Diagnostics reserves the right to modify or update these Terms at any time. Any changes will be posted on our website and will be effective immediately upon posting. It is your responsibility to review these Terms periodically to stay informed about any updates.
12. Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the laws of Maine. Any disputes arising from the use of our services shall be subject to the exclusive jurisdiction of the courts located in Maine.
