Simple Three-Step Referral Pathway

Complete the Patient Referral Form

Download the form below, fill it out per patient with CPT code(s), ICD-10 indication, and your NPI. No prior authorization needed. No insurance pre-approval.

Patient Schedules — We Come to Them

Your patient books directly at blackpointdiagnostics.com/booking or by calling (207) 409-7797. The completed referral form can be brought to the appointment or emailed to Emanuel@blackpointdiagnostics.com (via our HIPAA-compliant email service). We travel to the patient's home, workplace, or a partner location — no hospital visit required.

Cardiologist Report Within 24–48 Hours

All images are reviewed and reported by Dr. Gandelman, board-certified cardiologist at Greenwich Cardiology. Written reports are emailed to the patient within 24–48 hours. A copy is sent directly to your practice upon request — simply indicate this on the referral form.
