Why Diabetes Needs Three Territories Checked

Diabetes does not damage one artery. It damages the lining of arteries throughout the body, quietly, over years. These are the three places where that damage does the most harm and where finding it early still changes the outcome.

The arteries to your brain

Diabetes accelerates plaque formation in the carotid arteries. A carotid Duplex with IMT measures both the plaque and the artery wall thickness — one of the earliest measurable signs, often visible years before a blockage.

The arteries to your kidneys

Narrowing of the renal arteries drives blood pressure that will not come down and accelerates kidney decline. A renal arterial Doppler measures the flow directly, which a blood test cannot do.

The arteries to your legs and feet

Peripheral arterial disease is the pathway to non-healing wounds and amputation. And in diabetes it is frequently silent — neuropathy can blunt the leg pain that would otherwise warn you.

That last point is why screening matters more in diabetes, not less. The usual warning sign for poor leg circulation is pain on walking. Diabetic nerve damage can remove that warning, so the disease advances without the symptom that would normally send someone to a doctor. Imaging does not depend on whether you can feel it.

Before you come

The renal study images through the abdomen, so it is best done fasting — nothing to eat for six hours beforehand, water and medications as normal. If you take insulin, tell us when you book and we will schedule around it.

The carotid scan — about 30 minutes

Warm gel and a small probe on each side of the neck. Entirely painless.

The renal arterial study — about 45 minutes

The probe moves across the abdomen and flank. You will be asked to hold your breath briefly so the kidney arteries hold still.

Both legs — about 45 minutes

Arterial flow measured from the groin to the ankle in each leg.

Frequently Asked Questions

My diabetes is well controlled. Do I still need this?

Good control slows vascular damage; it does not stop it, and the damage accumulates over years rather than showing up on a recent A1c. This package images the arteries themselves rather than inferring their state from blood sugar.

My feet do not hurt. Doesn't that mean my circulation is fine?

Not necessarily, and this is the most important thing on this page. Leg pain on walking is the usual warning sign of poor arterial flow — and diabetic neuropathy can remove it. Absence of pain in a diabetic foot is weaker reassurance than it would be in anyone else.

Why the kidneys? I already get blood work.

Blood tests show how the kidneys are performing. They do not show whether the arteries feeding them are narrowed. Renal artery narrowing is a treatable cause of blood pressure that resists medication, and it is invisible to a blood panel.

Do I need a referral in Maine?

How long does it take, and do I need to fast?

About two hours for all three studies. Please fast for six hours beforehand so the renal arteries can be imaged clearly — water and medications are fine.
