Why Swelling Needs Three Scans, Not One

Swollen legs and ankles have more than one cause, and they look identical from the outside. Telling them apart is the whole point of this package.

The heart is not pumping strongly enough

When the heart weakens, fluid backs up into the legs and lungs. An echocardiogram measures the pumping function directly — ejection fraction, chamber size, valve function.

The vein valves in the legs have failed

Healthy leg veins have one-way valves. When they leak, blood pools and the ankle swells — with a completely normal heart. A venous Doppler sees the valves working, or not.

A clot is obstructing the vein

A deep vein clot causes swelling too, and it is the one cause that is urgent. The same venous scan rules it in or out, in both legs.

A heart scan alone cannot tell you which of these it is. An echocardiogram showing a normal pump does not explain the swelling — it only removes one of three explanations. This package examines all three in one visit, and the carotid study adds the downstream question: heart failure raises clot risk, and the carotid arteries are where that risk reaches the brain.

What Happens

Your Visit, Step by Step

About an hour and a half, start to finish. No needles, no radiation.

Before you come

Eat normally, take your medications as usual. Wear something easy to change out of — the echo needs chest access and the venous study needs both legs.

The echocardiogram — about 45 minutes

You lie on your left side. The probe moves across the chest and under the ribs, with a few short breath-holds. You can watch your own heart on the screen.

The carotid scan — about 30 minutes

Warm gel and a small probe on each side of the neck. You will hear your own blood flow through the Doppler.

Both legs, groin to ankle — about 45 minutes

The probe presses gently along the length of each leg — that compression is how a clot is found. Valve function is tested by having you breathe or bear down.

A cardiologist reads all three

Board-certified, in a lab holding IAC accreditation in Echocardiography and Vascular Testing. Read together, so the heart findings inform the leg findings.

My ankles swell but my doctor said my heart is fine. Is this still worth it?

That is exactly the situation this package is built for. A normal heart removes one of three explanations and leaves two unexamined — failing vein valves and a deep vein clot both cause the same swelling with a perfectly normal echocardiogram. The venous study is what answers the remaining question.

Why is a carotid scan included for leg swelling?

It is not for the swelling. A weakened or irregular heart raises the risk of clots forming and travelling, and the carotid arteries are the route to the brain. Since the echocardiogram is already being performed, examining the downstream consequence in the same visit costs 30 minutes rather than a separate appointment.

Is this the same as the test my doctor would order for a clot?

The venous portion is the same study — a bilateral lower extremity venous duplex, performed by an ARDMS-certified sonographer and read by a cardiologist. If you have sudden, painful, one-sided swelling, do not book a screening: go to an emergency department today.

Do I need a referral in Maine?
