Why Flying Raises Your DVT Risk
When you sit still for several hours — legs bent, feet on the floor, circulation slowed — blood can pool in the deep veins of your calf. This is the environment in which blood clots form. Flights over 4 hours carry the highest risk, but even shorter flights can matter if you have other contributing factors. This is not rare: research estimates that roughly 1 in 4,656 long-haul flights results in a DVT that requires treatment — and many more form silently without immediate symptoms.
What Could Be Causing Your Calf Pain
1 — Deep Vein Thrombosis (DVT)
DVT occurs when a clot forms in a deep leg vein — most commonly in the calf or thigh. The clot obstructs normal blood flow, causing swelling, pain, warmth, and sometimes redness. The concern is not just the local discomfort: a clot can detach and travel to the lungs (pulmonary embolism), which can be life-threatening. A venous duplex ultrasound can detect or rule out DVT directly and non-invasively.
2 — Muscle Strain from Cramped Seating
Hours in a confined seat, with knees bent and legs unable to fully extend, can cause muscle tightness and soreness in the calf — particularly the gastrocnemius or soleus muscle. This discomfort typically improves within a day or two with movement, stretching, and hydration. Unlike DVT, muscle strain is usually symmetrical and resolves predictably. If only one calf is affected and the pain is persistent, DVT is the more likely explanation.
3 — Dehydration-Related Cramping
Airplane cabin air is extremely dry, and alcohol consumption during flights compounds fluid loss. Dehydration reduces blood volume and can trigger painful muscle cramps, particularly in the calves. These cramps are usually brief, respond quickly to hydration and electrolytes, and do not cause persistent warmth or swelling. Persistent symptoms after rehydrating warrant further evaluation.
Your DVT Risk Factors After Flying
Your risk is higher if any of the following apply:
- Flight duration over 4 hours
- Recent surgery or hospitalization (within the past 3 months)
- Use of oral contraceptives or hormone replacement therapy
- Personal or family history of DVT or pulmonary embolism
- Active cancer or recent cancer treatment
- Pregnancy or recent postpartum period
- Obesity (BMI over 30)
- Age 60 or older
