Fatigue and lower-leg swelling together, without a diabetic explanation, tend to point in a cardiac or venous direction. Here are the four most important causes to evaluate:
Heart failure doesn't mean the heart has stopped — it means the heart isn't keeping up with the body's demands. When the left ventricle's ejection fraction falls, less blood is pumped out per beat, and the body receives less oxygen-rich blood. The result is persistent fatigue that doesn't respond to rest. Simultaneously, fluid backs up into the body's tissues, pooling in the legs and feet by end of day. An echocardiogram measures ejection fraction directly and can help identify whether reduced pump function is contributing to symptoms.
Heart valves control the direction of blood flow through the heart. When a valve becomes leaky (regurgitation) or too narrow to open fully (stenosis), the heart must work harder to maintain circulation. Significant aortic stenosis or mitral regurgitation can produce fatigue and exercise intolerance as early symptoms — often years before more dramatic symptoms appear. An echocardiogram evaluates all four valves and can help identify the severity of any dysfunction.
Pulmonary hypertension — elevated pressure in the lung's blood vessels — forces the right side of the heart to work harder than it was designed to. Over time, the right ventricle can enlarge and weaken, causing fluid to accumulate in the body (right heart failure). Symptoms include fatigue, swollen legs, and shortness of breath with exertion. An echocardiogram can estimate pulmonary artery pressures and evaluate right ventricular size and function, and can help identify this condition before it becomes severe.
If the heart checks out normally, the next place to look is the leg veins. Venous insufficiency — where one-way valves in the veins fail and blood pools — causes swelling, heaviness, and aching in the lower legs. The fatigue associated with venous insufficiency tends to be localized to the legs rather than systemic, and it's typically worse at end of day. A venous duplex ultrasound maps valve function and can help pinpoint exactly where the reflux is occurring.
