The medical term is amaurosis fugax — transient monocular vision loss. People describe it as a curtain or shade dropping over one eye, or a sudden graying out that resolves within seconds to minutes. It is painless. It passes. And it is one of the most important warning symptoms in cardiovascular medicine, because it indicates that something — typically a micro-embolus from the carotid artery — just temporarily blocked blood flow to the eye's retinal artery. The embolic source is still present. The question is what it is and how significant it is.
What Could Be Causing This
1 — Amaurosis Fugax from Carotid Plaque
The most common cause of transient monocular vision loss is atherosclerotic plaque in the ipsilateral carotid artery — the carotid artery on the same side as the affected eye. The carotid artery branches into the ophthalmic artery, which supplies the retina. Plaque can release tiny fragments (micro-emboli) that travel up through this pathway and temporarily occlude retinal blood flow. When the embolus dissolves, vision returns. Even without a vision episode, significant carotid stenosis carries substantial stroke risk. A carotid duplex ultrasound can directly visualize the artery wall, quantify plaque burden, and measure the degree of stenosis — providing the data needed to determine whether intervention is warranted.
How to Find Out for Sure
A Carotid Duplex Ultrasound is the first-line imaging study after a vision episode of this type. It directly images both carotid arteries, identifies plaque, and measures stenosis severity. B-mode imaging reveals the artery wall and plaque morphology; Doppler measurements quantify blood flow velocity, which correlates with the degree of narrowing.
