Clinical Indications

When Your Doctor Recommends an AAA Screening

An abdominal aortic aneurysm (AAA) screening is a focused ultrasound of the largest artery in your body — the aorta — as it passes through your abdomen. An aneurysm is a dangerous bulge in the aortic wall that can expand silently over years and rupture without warning. A ruptured AAA is fatal in up to 80% of cases. The scan is quick — approximately 15 minutes — painless, and requires no preparation. If you are a man over 65, have a history of smoking, or have a family history of aortic aneurysm, screening is strongly recommended by the U.S. Preventive Services Task Force.

AAA rarely produces symptoms until it reaches a dangerous size or ruptures. Patients occasionally report deep abdominal pain, back pain, or a pulsating sensation near the navel, but most aneurysms are discovered incidentally or through proactive screening. Risk factors beyond age and smoking include high blood pressure, atherosclerosis, family history of aneurysm, and connective tissue disorders such as Marfan syndrome. Women with a smoking history are also at elevated risk. Early detection through screening allows for monitoring and planned intervention — a stark contrast to emergency surgery after rupture.

Saco residents have limited options for AAA screening without entering the hospital system. The nearest hospital-based labs are at Southern Maine Health Care (Biddeford) — both requiring a physician referral, insurance pre-authorization, and often a multi-week wait for scheduling.

What We're Looking For

What an AAA Screening Evaluates

Aortic Diameter

Measures the width of your abdominal aorta. Normal is under 3 cm. An aneurysm is diagnosed at 3 cm or greater, with surgical referral typically considered above 5.5 cm.

Aneurysm Shape

Identifies whether an aneurysm is fusiform (symmetrical widening) or saccular (bulging on one side). Shape influences rupture risk and treatment decisions.

Growth Rate Monitoring

For known aneurysms, serial ultrasound tracks growth over time. Rapid expansion (>0.5 cm in 6 months) accelerates the timeline for intervention.

Mural Thrombus

Detects blood clot formation along the aneurysm wall. Thrombus can embolize to the legs or indicate an unstable aneurysm.

Iliac Artery Extension

Evaluates whether the aneurysm extends into the iliac arteries below the aorta — a finding that changes surgical planning.

Surrounding Anatomy

Assesses the relationship of the aneurysm to the renal arteries and other branches, which determines whether endovascular repair is feasible.
