The carotid arteries are the primary vessels supplying blood to the brain. When atherosclerosis takes hold — the same plaque process responsible for heart attacks — it progressively narrows these vessels and creates unstable deposits along the arterial wall. Those deposits can fracture. When they do, a fragment travels upstream to the brain, blocking flow and triggering a stroke or TIA within seconds.
A transient ischemic attack — commonly called a mini-stroke — is the body's clearest warning signal that significant carotid disease may already be present. It should never be dismissed. A carotid duplex ultrasound is the established diagnostic test for evaluating this risk. It visualizes plaque directly, measures how much the artery is narrowed, and characterizes whether the plaque is stable or vulnerable — critical information for determining whether intervention is needed before a stroke occurs.
Carotid artery disease develops silently over decades. Most patients have no symptoms until the moment a stroke or TIA occurs. Screening is most valuable for individuals in higher-risk categories — where the likelihood of finding actionable disease is significant.
A carotid duplex ultrasound uses real-time Doppler imaging to evaluate blood flow velocity and direction through both carotid arteries. The study measures the degree of stenosis — classified as mild, moderate, or severe — and provides direct visualization of plaque morphology. Stable calcified plaque behaves differently from soft, lipid-rich vulnerable plaque, and that distinction directly shapes clinical decision-making.
The results of this scan inform whether watchful waiting, aggressive medical management, or surgical consultation is appropriate.
